Respire Homecare is contracted with most insurance companies and managed care organizations to provide portable medical ventilators, home oxygen, CPAP, and other respiratory equipment. As a Medicaid contract provider, we cater to almost all respiratory patients with complex diseases, thanks to our 46 years of combined respiratory experience.
How Do I Refer a Patient to Respire Homecare?
To refer a patient to Respire Homecare, simply give us a call at (877) 255-2234 or (330) 722-1711! One of our customer service representatives in the intake department will help coordinate the referral.
Information We Need
- The patient’s medical history and diagnosis
- The patient’s insurance information
- The patient’s home and social status, including family and caregiver support
Documents We Need
- The prescription (including settings if required)
- History and physical
- Documents needed for insurance authorization such as sleep studies, ABGs, and any other pertinent lab results
Referrals for Patients That Are Already Home
After you have faxed the referral documents, we will immediately contact the patient about the referral. Upon insurance verification and/or authorization, we will schedule a setup at home, an appointment at our office, or arrange for shipment to the patient’s home.
Referrals for Patients Pending Hospital Discharge
After you have faxed the referral documents, we will immediately contact the patient or the patient’s family about the referral. We will work closely with you and the patient to make arrangements for equipment training and setup.
